The US Food and Drug Administration (FDA) on Friday granted emergency use authorization (EUA) to Pfizer and BioNTech’s COVID-19 vaccine candidate, following a vote by the agency’s independent advisory panel last Thursday recommending authorization. Results from the vaccine’s phase 3 clinical trial released in late November showed 95 percent efficacy of the two-dose regimen, leading the companies to submit the candidate to regulatory bodies around the world. The candidate, the first mRNA-based vaccine ever to receive regulatory authorization, is the first COVID-19 vaccine to receive a green light in the United States and has also been authorized in the United Kingdom, Canada, and several other countries. The EUA allows the United States to begin vaccinating people 16 years of age and older in a massive distribution effort prioritizing health care personnel and residents and staff of long-term care facilities.As COVID-19 vaccine approval and rollout becomes a reality, a group of global health organizations is warning that low-income countries will face extreme challenges with vaccine access due to high-income countries securing a vast majority of the global vaccine supply through advance purchase deals with vaccine developers. Data on these agreements and the current vaccine pipeline indicates that nearly 70 low- and middle-income countries will be only able to vaccinate 10 percent of their populations against COVID-19 in the next year. A recent study also outlines the confirmed purchases of at least 6.8 billion doses by primarily high-income countries, creating a risk that lower-income countries will be forced to wait for doses, deepening existing inequities and furthering economic damages. Low-income countries will likely be reliant on doses provided through the COVAX Facility, a global mechanism to speed up development, manufacturing, and equitable distribution of COVID-19 vaccines—an initiative researchers say is being undermined by these unequal vaccine deals.Efforts to develop COVID-19 treatments are moving forward as companies look to add to the arsenal of therapeutics helping to minimize the effects of COVID-19 illness. Since the onset of the pandemic, FDA has authorized a few treatment options, including remdesivir, convalescent plasma, and monoclonal antibodies. Other therapeutics remain under development, including an apelin agonist peptide treatment developed by CohBar, which recently demonstrated efficacy in preclinical models to treat acute respiratory distress, and Sorrento Therapeutics’ intravenous STI-2020, which recently received FDA clearance to begin phase 1 trials as a potential early treatment for COVID-19 outpatients with mild symptoms. Both developers plan to accumulate further safety and efficacy data of the therapeutics in the upcoming months.

The United Kingdom has a supply deal for 40 million doses of the vaccine, enough for 20 million people, which it will begin to distribute in the upcoming days to priority groups including care home residents, health and care staff, the elderly, and populations that are clinically extremely vulnerable. The mRNA-based vaccine, which was shown to have 95 percent efficacy in phase 3 trials, requires a strict cold chain and will be delivered through a network of 50 hospitals, as well as specialist vaccination centers, and any general practitioners and pharmacists that have cold storage capacity. The US Food and Drug Administration (FDA) and the European Medicines Agency are expected to finalize their reviews of the vaccine candidate within the next few weeks.A major federal advisory committee in the United States voted Tuesday that health care personnel and people who live in long-term care facilities should receive priority in COVID-19 vaccine distribution. However, the Advisory Committee on Immunization Practices (ACIP) also acknowledged that pregnant health care workers, who make up about 330,000 members of the workforce, are at heightened risk of COVID-19 mortality and that none of the initial vaccine products have been specifically tested in pregnant people, leaving their inclusion in early vaccine rollout an open question. The unclear implications of the guidance likely means that if the ACIP recommendation is accepted by the Centers for Disease Control and Prevention, the decision will fall to individual states and health care systems to determine whether to offer pregnant health care workers the vaccine alongside their non-pregnant colleagues. FDA emergency use authorization applications for Pfizer’s and Moderna’s COVID-19 vaccine candidates are currently pending, which could lead to approval and rollout later this month.Biotech company Roche received FDA emergency use authorization for its COVID-19 antibody blood test capable of measuring the levels of specific antibodies that target the virus’ cell-unlocking spike protein. The quantitative antibody test provides a result in about 18 minutes and was shown to be very accurate in the 15 days after screening and diagnosis of an active infection with a molecular test. Studies also showed that patients’ antibody concentrations did not fall below a detectable range for at least 100 days following infection. The company has said tests of this kind will play a critical role in measuring vaccine-induced immune responses, and hopes it will help track the success of vaccines as well as identify people who have recovered from an infection and could serve as convalescent plasma donors or assist in the development of new therapies.

The updated results from Moderna’s 30,000-volunteer phase 3 study show a total of 196 symptomatic cases of COVID-19, of which 185 received a placebo injection and 11 received the vaccine. The news follows similarly strong results from Pfizer and BioNTech’s vaccine candidate, which the companies submitted for US Food and Drug Administration (FDA) emergency use authorization on November 20. The FDA committee will convene December 10 to review Pfizer’s vaccine candidate and December 17 to review Moderna’s candidate.Thirteen African countries and an international network of research institutions are launching a multi-country clinical trial of COVID-19 treatments in patients with mild to moderate illness. Led by the Drugs for Neglected Diseases initiative, the ANTICOV trial aims to identify drugs that can be used for early treatment of COVID-19. The ANTICOV consortium will carry out the trial at 19 sites across 13 countries and will begin by examining repurposed drugs that have previously been used for HIV, malaria, and other infectious diseases before expanding to additional treatments and adapting over time. The trial is part of the Access to COVID-19 Tools (ACT) Accelerator therapeutics pillar, launched in April to ensure equitable global access to COVID-19 innovations.As several COVID-19 vaccine candidates await regulatory clearance by health authorities around the world, the question of global distribution efforts is becoming more relevant. While the United States and European countries have signed direct agreements with vaccine developers to secure enough doses to cover their populations if the vaccine candidates are approved, many low- and middle-income countries (LMICs) will be reliant on the ACT Accelerator’s vaccines pillar, a mechanism aiming to accelerate development and equitable distribution of COVID-19 vaccines. The effort plans to distribute 1 billion vaccine doses to LMICs based on a “fair allocation program” by the end of 2021, which will cover about 20 percent of their populations, but it will likely face distribution and storage challenges due to funding gaps, manufacturing facility locations, and cold-storage requirements.

A first look at early phase 3 data had showed the mRNA-based vaccine was more than 90 percent effective at preventing COVID-19 infection; follow-up numbers released after the trial collected sufficient safety data then showed a 95 percent efficacy rate. The vaccine could receive authorization by the end of the year—allowing distribution to begin in 2020—depending on the speed of the review, according to the CEO of BioNTech. The vaccine developers expect to produce up to 50 million doses of the two-shot regimen in 2020 and up to 1.3 billion doses in 2021.New data from AstraZeneca and the University of Oxford’s phase 3 COVID-19 vaccine trial signaled that the candidate has an average efficacy of 70 percent, with a range of 62 to 90 percent in different dosing schemes. Late last week, AstraZeneca and Oxford released results from their phase 2 trial, which showed that the candidate was safe and produced a strong immune response in younger adults and older participants. The developers plan to file for emergency use from regulatory bodies in the United Kingdom and other countries and expect about 3 billion doses to be ready in 2021. The results from Oxford/AstraZeneca’s trial followed announcements by two US-based vaccine efforts—Pfizer/BioNTech and Moderna/National Institute of Allergy and Infectious Diseases—indicating more than 90 percent efficacy of their COVID-19 vaccine candidates in phase 3 trials.FDA has granted EUA to the first rapid COVID-19 diagnostic able to be performed entirely at home. The test, developed by Lucira Health, requires the user to self-conduct a nasal swab, which is then swirled in a vial of laboratory solution that plugs into a portable device. The rapid result—which is delivered within 30 minutes—is then displayed as a light, labeled positive or negative. Initial use of the single-use test kit will require a prescription, but the company has said it expects the test to be widely available by next spring at a cost of US$50. FDA has authorized emergency use of nearly 300 COVID-19 diagnostics to date, but this will be the first conducted entirely at home, which experts hope will allow quicker quarantining and contact-tracing to combat spread of the virus.

The companies hope to have enough data to file for emergency use authorization (EUA) within the next few weeks. If the vaccine candidate does receive approval, Pfizer and BioNTech will launch their extensive distribution plan—which requires the vaccine to be kept at -94°F, a strict cold chain that has led to feasibility concerns from many countries—to fulfil hundreds of millions of doses ordered through advanced purchase commitments. These commitments include a 300-million dose deal with the European Union, a 120-million dose deal with Japan, a 100-million dose deal with the United States, and a 30-million dose deal with the United Kingdom. Pfizer and BioNTech plan to roll out a total of 100 million doses of the vaccine by the end of 2020, followed by an additional 1.3 billion doses in 2021.The US Food and Drug Administration (FDA) last week granted EUA to Eli Lilly’s COVID-19 antibody treatment for use among people age 12 or older with mild or moderate COVID-19 infection. The drug, bamlanivimab, continues to be tested in a clinical trial to establish additional safety and efficacy data, after early results suggested it may help clear COVID-19 infection sooner and cut hospitalization rates in patients with mild or moderate cases. A study of the treatment in hospitalized patients was stopped in late October due to lack of efficacy, upon recommendation from independent monitors of the trial. The US government has signed an agreement with Eli Lilly to purchase 300,000 700-mg vials of the drug, though it is unclear how many doses that will provide, as early results indicated at least 2,800 mg is necessary to provide any effect. Eli Lilly’s therapeutic is the first antibody drug to receive EUA to treat COVID-19.The Senegal-based Pasteur Institute is developing an affordable, handheld COVID-19 diagnostic kit able to provide results in minutes. The rapid test kit contains a simple test strip housed in a plastic unit and requires a small blood sample, collected by finger prick. No electricity or laboratory analysis is required, and the center hopes the kits will cost as little as US$1. Developers plan to start manufacturing and distribution after regulatory checks are complete, making the kits first available to countries in Africa through governments and health organizations like the Africa Centers for Disease Control and Prevention, with a goal of issuing 10 to 15 million kits by February 2021.

The mechanism—one of four ACT Accelerator pillars working to scale COVID-19 health technologies—is co-led by Unitaid and the Wellcome Trust and is dedicated to securing and supplying COVID-19 therapeutics worldwide with an emphasis on equitable distribution. The scheme’s list of priorities outlined in the draft document says efforts will focus on advancing research and manufacturing of monoclonal antibodies and boosting distribution of the steroid dexamethasone, and does not mention remdesivir among priority drugs. The therapeutics pillar is also still working to reach its US$7.2 billion funding goal, $6.1 billion of which is unmet, and aims to distribute hundreds of millions of COVID-19 treatment regimens to lower-income countries by 2022.A recent analysis from researchers at Duke University’s Global Health Innovation Center found that high- and middle-income countries have purchased 3.8 billion COVID-19 vaccine doses, with options for 5 billion more, potentially leaving people in low-income countries without access until 2024. The study suggests that these direct deals between countries and drug manufacturers, known as advance purchase agreements, are exacerbating “vaccine nationalism” and undermining the equitable distribution efforts of mechanisms like the COVID-19 Vaccines Global Access (COVAX) Facility, a sub-mechanism of the ACT Accelerator. More than 150 countries, including many that have also made separate country-specific vaccine deals, have signed on to join the COVAX Facility, which aims to develop and equitably distribute 2 billion vaccine doses by the end of 2021.A nasal spray under development to prevent COVID-19 infection was shown to be effective in blocking absorption of SARS-CoV-2 in ferrets exposed to the virus, according to a research team from Columbia University Medical Center, Erasmus Medical Center, and Cornell University. The spray is designed to intervene in the virus’ process of attaching to airways or lung cells by fusing itself to an amino acid chain on the virus’ spike protein, thereby blocking viral attachment. In the very small animal study, the spray prevented all direct-contact transmission of SARS-CoV-2 during a 24-hour exposure period, indicating potential for the product to protect against COVID-19 infection in humans, although the research is still preprint and pre-peer review. Based on the promising animal results, researchers hope to secure additional funding that would allow them to expand tests of the nasal spray into human trials.
